Once you fill out the very basic information on our Request Appointment page, our office will be in touch with you to confirm the details of your appointment, including answering any questions regarding insurance coverage. No payment will be processed until you are seen in our office.

Also, requested appointments are not guaranteed until you hear back from our office that the appointment has been confirmed. If you are a new patient, the appointment will not be confirmed until you speak with the office administrator and may be declined due to a variety of reasons. In this event, you will receive a call and text from the office for you to speak with the office administrator.

You will hear from our office within 48 hours unless it is a Friday/weekend or holiday after you request an appointment. If not, you can call us at 970-828-6500
